Frequently calls reach us from relatives who are looking for relief. But courtly appointed caretakers and social workers of the hospitals need for their patients adequate care facilities. In our schematized representation, we show hereafter, in wich way a first interview and the other procedure could proceed.

 
  • Phonecall to the AABK by relatives, the courtly appointed caretakers, social workers of the hospitals
  • What is wanted? Outpatient care, inclusion in the day-care center or assisted living?
  • An employee of AABK makes a first appointment for an informational discussion. Visitors to the day care center should be able to walk.
  • Visit the day care center and presentation of the daily routine: Which reactivating measures are used, information on costs, funding opportunities, help with the application or acquisition of funds for the care or social services.
  • Mediation for the outpatient services. The business office is on the Werdener Street 4. in 40227 Düsseldorf. 
  • In a second date: Information collection on the new interested, interviews with relatives, social workers, doctors and other affected people. Formulation of the biography, history and anamnesis. How are the current living conditions, habits, dislikes, medical diagnoses, behavior, character and social environment?
  • By joining the day-care centre both sides agree on a trial-day: The visitor learns know us and we get to know him. If we adjust to each other, a contract will be made. The appropriate form can be obtained from the AABK. The contract can be closed to every time by the customers.
  • Then: An individual care plan will be coordinated.
